We are now registering new patients.
In order to register you will need to fill in a registration form, available at Reception, and bring photographic ID and proof of address.
Please note our practice area has changed, the map is available on this site and also in reception.
Central Surgery has a strict registration Policy, in compliance with NHS Regulations, which states that if a Patient moves out of the Practice Area the patient will not be permitted to stay on our Practice list and will have to register at a New Practice. This is to ensure that the Practice remains non-discriminatory and to be fair to all Patients who remain in the Practice's registration area.
